The best I've seen by miles has to be the first nights broadcasting of live sports xxx babes (called something like that).

When I realised that Hannah Claydon and Hannah Mac were two different women...
I thought the names on screen were wrong showing C and M. Only noticed the weren't the same person when they were on at the same time. Couldn't believe it!

Ok I will add a few things to this thread, good reading btw
